Please contact our office for a list of insurances we accept and to see if we are accepting new patients at the time of your call.

We do not accept Medicare or MediCal at this time.

Dr. Upadhyay also sees Private Pay patients. If Dr. Upadhyay does not take your insurance, you may be able to see her as an “out- of- network” provider. She can provide you with a “superbill” which you can submit to your insurance company for reimbursement. Please contact your insurance company ahead of time to find out if you have any “out-of-network” benefit.

We recommend contacting your insurance company and asking about benefits prior to scheduling an appointment.

Most insurance companies do not reimburse for missed appointments.

Please check with your insurance company to see if they will reimburse for Telepsychiatry sessions prior to booking a Telepsychiatry appointment.

Payment is due at the time of service. Forms of payment include cash, check or debit card. Credit cards may also be processed with a 2.5% processing fee.

Please note that Dr. Upadhyay will attempt to complete the majority of documentation during your appointment time.

All medical forms (such as disability forms, school forms etc.) will be completed by Dr. Upadhyay during your appointment. Please notify her at the beginning of the appointment if you have forms to be filled out.

CANCELLATION / MISSED APPOINTMENTS/ LATE ARRIVAL/ NO SHOW POLICY:

2 Business Days notice is required for all initial assessments. A missed appointment fee of $200 will be applied to any cancellation for initial assessment that is made with less than 2 business days notice.

2 Business days notice is required if you cannot make a follow-up appointment. A missed appointment fee of $75 will be applied for any no-shows or for cancellation made with less than 2 Business days notice.

Please note if you arrive late to your appointment, Dr Upadhyay can see you only for the time left in your appointment session.

Missed, cancelled or late visits interfere with the quality of patient care, result in longer wait times and create a financial burden for the practice.
